Membership Pricing:
Our membership fee covers enhanced access, personalized care, and wellness planning. To provide this elevated level of care, we offer our concierge membership for:
- $2,500 per year for individuals
- $4,897 per year for couples
- Family Plans available and dependents under the age of 26 are free with an active parent plan.

Our Services
Insurance
Our Personalized Care Model works in conjunction with your private insurance. Because of this, we continue to bill your insurance for covered medical services, combining the benefits of personalized attention with traditional healthcare coverage.
We participate with most major insurance carriers, however, some specific policies under that carrier may be under a network that we are not contracted with. The best way to ensure we are an in-network provider is to call the customer service line listed on the back of your insurance card to verify that either Dr. Kristine Sarna or Dr. Rebecca Weiss is listed as an in-network provider for your specific insurance policy. We recommend that all patients verify this very important information before you schedule your appointment to prevent any unexpected amount due for your office visit.
